Roles & Responsibilities
Position Overview :
As a Social Worker, you will be working directly with youths and their families within the community. Your focus will be on providing meaningful interventions, strengthening reintegration efforts, and fostering positive youth development through collaborative case management and stakeholder engagement.
Key Responsibilities (not limited to) :
Conduct case orientation and develop tailored intervention plans from the outset.
Use appropriate frameworks to set clear, achievable goals and review progress periodically.
Work closely with Probation Officers (POs), Child Protection Officers (CPOs), schools, families, and employers for joint case management and holistic care.
Build and maintain partnerships with schools, employers, social service agencies, and other community stakeholders to support the youth’s reintegration.
Attend and present at various meetings and review platforms to provide updates on intervention progress.
Maintain accurate and timely case notes and documentation in accordance with organizational guidelines and standards.
Liaise with the Hostel Operations & Development Department to ensure accurate information is provided for daily scheduling and movement planning.
Conduct and participate in group-based interventions or activities for youth residents.
Support ad-hoc tasks and initiatives as assigned by supervisors or management.
