Work with pyschologist and physiotherapist to conduct therapeutic and social interventions for the Adult Disability Home (ADH) Residents and the Day Activity Centre (DAC).
Monitors the progression of annual Individual Care Plan (ICP) goals; and document the completion of ICP interventions for the ADH Residents and DAC Clients.
Assists to develop and expand the current pool of therapeutic resources and task materials of the Home and the DAC.
Optimizes the social, emotional, cognition, Activities of Daily Living (ADL), and Community Living Skills (CLS) performance through the use of individualized right-fit and right -challenge interventions.
Monitors, completes, and reports on the annual outcome measure of ADH Residents with the Quality of Life (QoL) tool monitor and track individual Residents' progress.
Liase with the assigned volunteers and trains them on the handling and supervision of manual and motorized wheelchairs, as well as community mobility to the neighborhood locations.
Engages the ADH Residents in interactive virtual games, conventional board games, physical exercises, money management activities, purposeful occupational tasks, and community mobility activities.
