Job Description
1. Support team in monitoring of seniors' physical and mental health and well-being :
- Conduct visual check and take / monitor vital signs
- Act on clients' irregular conditions, report to senior care staff or supervisor and caregiver, if necessary, record in communication notes, submit incident report where needed
- Identify changes in seniors' well-being / behaviours, report to caregivers, plan and monitor Individualised Care Plan (ICP) with Physiotherapist or Occupational Therapist.
- Respond to emergency and accidents. Use of PPE if required
- Administer medications following SOP, after being trained
2. Provide high level of customer service and engagement :
Plan and lead in exercises and activities for the seniorsServe meals and teabreaksProvide personal care to seniors to carry out their routine activities, being attentive to their needs and ensure their safety in the centre.3. Ensure safety and cleanliness :
Assist seniors in toileting.Mobility and transfer safety after being trainedFollow infection control procedures, maintain cleanliness and safety of the premiseProvide support as a transport escort to send and fetch clients to and from centre, if BCLS-trained.Qualifications
Minimum 1 year of working experience in healthcare / eldercare setting is preferred or possess any of the following :
Diploma in Healthcare Management or GerontologyWSQ Diploma in Social ServiceSGUS programme for Care and Coordination (Community Eldercare)WSQ Certificate in Healthcare (Therapist Support or Nursing)Requirements :
Only SingaporeansPassionate and patience in serving the seniorsRequired to attend to seniors who are mostly mandarin / dialect-speaking. Proficiency in mandarin and / or dialects is essentialHumble, willing to learn, team playerSelf-driven, willing to take on challengesAble to drive is an added advantage.
