Job purpose
The jobholder is primarily responsible to cultivate a vibrant community of senior volunteers, encouraging active participation among seniors and fostering a supportive atmosphere for community involvement. He / she is to provide support to seniors through engaging activities, home visits, promoting social interaction and active ageing. He / she is to facilitate outreach initiatives to invite seniors to join our Active Ageing Centre, facilitating social engagement and personalized care. He / she will also be required to support the Active Ageing Centre Department with ad-hoc and secretariat duties.
Duties and responsibilities
- Facilitate Active Ageing Programmes (AAPs) and engage seniors via calls and visits to participate in the activities. Attendance should be registered in the system with every engagement conducted.
- Take charge of Community Screening (CS), ensuring seniors' needs are screened and particulars registered and updated. Seniors' needs should be rescreened upon discharge from the hospital or renewed every 2 years.
- Ensure Befriending and Buddying seniors are engaged in AAPs on a monthly basis.
- Assist seniors in any Information and Referral requests and log in the requests in the system.
- Collaborate with grassroots, partners and vendors on AAPs and events.
- Any other duties assigned by superior and reporting officer.
Qualifications & Skills Required
Possess a diploma or higher (no preferred specialisation)IT savvy and proficient in Microsoft office applicationsKnowledge of programme design, implementation, and evaluationStrong interpersonal and communication skillsAbility to work independently and as part of a team, and to manage multiple tasks and prioritiesPreferably with relevant experience in working with elderly clients in the community setting and healthcare or social service sectorFresh university / polytechnic graduates are welcome.
