As a Community Care Associate at CWA, you will provide both supervisory and hands-on care responsibilities. You will oversee a team of Community Caregivers, support our home-based nursing operations, and deliver direct patient care when necessary. You will also assist in the review of clients and help ensure that care plans remain relevant and effective.
Key Responsibilities :
1. Direct Hands-On Personal Care
- Assist clients with activities of daily living (ADLs) including bathing, grooming, toileting, dressing, feeding, and mobility.
- Perform basic health monitoring such as checking vital signs (e.g. temperature, blood pressure, glucose levels).
- Support clients with prescribed exercises or rehabilitation activities as guided by allied health professionals.
- Maintain a clean, safe, and comfortable environment for clients.
- Provide companionship and emotional support to enhance client well-being.
2. Home Based Care (Support role)
Supervise and support a team of Community Caregivers (CCGs) in delivering consistentand quality care.

Participate in team meetings, case discussions, and review sessions.Job Requirements and Qualifications
Minimum WSQ Certificate in Healthcare Support (Home Care) or NITEC in Community Care & Social Services or ISC in Healthcare (Home Care).Prior experience in a healthcare or caregiving role is preferredStrong interpersonal and communication skillsBasic knowledge of healthcare practices and proceduresComfortable working in clients' homes for supervision and assessmentsProficiency in Microsoft Office (Word, Excel and Power Point).Able to use basic digital tools for care documentation (e.g., tablets, mobile apps).
