Roles & Responsibilities

  • The Social Work Assistant / Associate will provide psycho-social assessment, social work intervention, and support to clients and their families in resolving personal, social, emotional, and financial issues.
  • This role focuses on client well-being through regular visits or telecare, showing care and concern while fostering connections with available resources and services.

Duties and Responsibilities

Client Visitation and Telecare

  • Conduct home visits or telecare sessions to assess clients' emotional, social, and physical well-being.
  • Provide empathetic support and build rapport with clients and their families.
  • Accompany clients to other essential engagements when required.
  • Offer tailored advice on available resources and programs to meet individual needs.
  • Guide volunteers and acquaint them with clients during visitation or engagement activities.
  • Client Engagement

  • Follow up with non-active clients via phone calls or newsletters.
  • Motivate clients to participate in 365CPS programs, services, and community initiatives.
  • Administrative Support

  • Assist clients with applications for financial assistance and other welfare services offered by 365CPS.
  • Prepare detailed reports on client interactions, visitation updates, and progress.
  • Maintain an accurate client database for timely follow-ups and documentation.
  • Other Duties

  • Collaborate with the multidisciplinary team to enhance service delivery and client outcomes.
  • Organize and conduct support programs and activities.
  • Contribute to outreach events.
  • Job Requirements

  • Diploma in Social Work, WSQ Diploma in Community Care or Social Service, or equivalent qualifications (e.g., ITE Diploma).
  • Fresh graduates are welcome to apply; 1 to 2 years of relevant experience in the social service sector is an advantage.
  • A team player with excellent interpersonal and communication skills.
  • Patience, empathy, and strong listening skills to effectively engage and support clients.
  • Proficient in Microsoft Office applications.
