Join us as a Case Executive and be part of our CREST team.
You will need to
Provide care coordination and service linkage to clients with or who are at risk of mental health issues and / or dementia, with the appropriate resources and support including caregiver support
Plan and conduct outreach to the community and stakeholders including faith-based,corporate and community organisations to promote mental wellness and mental health awareness
Plan and conduct targeted mental health and dementia case finding / screening to promote early identification and facilitate timely support
Plan and organize structured preventive activities and / or programmes for the community, those at risk of mental health conditions and clients
Conduct regular visits to monitor the well-being of the elderly client (NTUCH CCMS / CREST)
Support the evaluation and documentation of the programme
Work with various organisations (i.e. regional health services, hospitals, volunteers, grassroots schools, and internal organisation departments) on activities and programmes for the seniors
Support other ad-hoc assignments at the discretion of the CREST Lead / Department head
Qualifications
Diploma / NITEC in community care & social services (without working experience)
Minimum 3 years experience in community healthcare settings in Singapore
Possess 1-2 years of administrative experience
You should
Have good knowledge of the geriatrics and community services
Be proficient in Google Suite applications
Have strong communication and stakeholder management skills
Be passionate about community services and be service-oriented
