Job description

Responsible for case work management involving systemic assessment and intervention for the well-being of patients and family members.

Perform referrals to other service providers and conducts home visit where necessary.

Work closely with multi-disciplinary team members.

Support projects and programmes.

Attend training and case work supervision.

Job Requirements :

Degree in Social Work or a Graduate Diploma in Social Work.
Registered with Singapore Association of Social Work.
Experience in means testing and healthcare setting is advantageous .
Salary range : $3400 - $6000
Working location : Toa Payoh
Working hours : Monday to Thursday : 8.00am to 5.30pm / Friday : 8.00am to 5.00pm
