Within our network of eldercare centres island-wide, the goal is to provide appropriate and affordable eldercare service to families who are in need of day care for their senior members and relatives.
We provide a suite of integrated services and programmes which include social day, dementia, nursing and respite care, along with Rehabilitation programmes such as physiotherapy and occupational therapy, wellness programmes and transportation. Through these services we hope to provide the finest service to the elderly in our care.
We are looking for a Staff Nurse (Home-Based) to join the team.
As a Staff Nurse (Home-Based), you will be responsible for performing patient visits and monitoring their conditions, conducting tests, and writing reports for the physicians. You should be able to address all the caregivers’ concerns and make appropriate adjustments to the patients’ in-home management plan.
Responsibilities :
- Conduct screening referrals and assessment of potential clients.
- Conduct home environment assessment.
- Develop an Individualised Care Plan (ICP) involving the caregivers and family members (if possible) for clients.
- Conduct a periodical review of clients’ ICPs and conditions and revise the ICP whenever necessary.
- Administering medication and insulin, and completing blood pressure, glucose, urine, and stool tests.
- Collaborating with doctors and other healthcare professionals to develop improved diets and healthcare plans for patients.
- Conduct health education and training to clients, caregivers as well as to the community.
- Testing for muscle weakness, bedsores, and any signs of infections.
- Provide basic care management’ service.
- Link and match clients to the required services.
- Provide advice on the application of government subsides.
- Collect payments and issues receipts for service delivered.
- Assist Doctors during the home visits when required.
- Participate in multi-disciplinary discussions.
- Develop discharge plan for clients who do not require the service.
- Discuss with caregiver regarding their needs in delivering the care to their loved ones and for self-care.
- Assist caregivers in navigating the eldercare and caregiver landscape.
- Link and match caregivers to the appropriate services.
- Assist with administrative work (e.g. data entry, generating of invoices & receipts, minutes taking, development and review of SOP).
- Provide support to new and less experienced staff.
- Participates in “Outreach and Road show” to promote the programme in the community.
- Develop close working relationships with other stakeholders (internal and external).
- Any other projects that are assigned by the supervisor.
