Roles & Responsibilities

Job Responsibilities

• Conduct intake assessment, coordination and facilitation of new referrals from various referral sources such as emails, phone calls and walk-ins
• Counsel family on programme fees, subsidy level
• Assess client’s financial situation to put up financial assistance application as required, including subsidy deviation & Medifund
• Provide individualized casework & psychosocial support to caregivers after identifying their needs, such as information & referral to assess appropriate care resources including financial schemes, psychoeducation on condition
• Co-develop individualized care plan for client and carry out 6 monthly review collaboratively with client, caregiver and other team members
• Involve in multidisciplinary care discussion with other team members
• Act as a point of contact between Centre, client / caregiver, and hospital / community stakeholders
• Conduct monthly review of means-testing for clients to maintain accurate records
• Organize caregiver support initiatives, such as workshops, and conduct yearly caregiver assessments and satisfaction survey to garner feedback
• Conduct home visits for client assessment or engagement, as required
• Help out in the Centre with operations and support with client supervision, as and when required
• Collate and compile client statistics and data, and submit accurate & timely reports to stakeholders, as required
• Ensure clients’ confidentiality is maintained and adhered to PDPA policy, when documenting via systems such as NMTS and CARES, or sending our clients details / information and when handling confidential information to stakeholders
