Job purpose :
Reporting to the Chief Operating Officer, the Senior Medical Social Worker is responsible for the overall social services, which seek to promote the psychosocial well-being of the residents and address the concerns of family members during the residents’ stay.
Key Responsibilites
- Psychosocial assessment - assessing strength and resilience of the resident, family, and social support systems to help the resident
- Counselling for residents and family members – for situations in which residents suffer from poor mental states (e.g. depression, anxiety) and coping and adjustment difficulties as well as financial issues
- Financial assessment and fund management - identifying and referring cases for financial assistance
- Referral Services and Discharge Planning – working together with medical, nursing and other allied health staff to : (i) assess new cases referred from Agency for Integrated Care or other sources for suitability, (ii) develop and implement the post discharge plan.
- Supervise team of medical social workers and social work assistants.
- Any ad-hoc duties as assigned.
Requirements :
Min Degree in Social Work / Post Graduate Diploma in Social WorkRegistered member of Singapore Association of Social Workers (SASW) preferredAt least 5 years of experienceSalary will commensurate with level of experience
