About the role
- 3 years contract - potentially convert to permanent upon completion
 - The Vocational Rehabilitation Specialist will support residents in Oasis Village at SANH (Taman Jurong) to engage in meaningful work and skill-building activities across its facilities, including the minimart, coffee shop, pet farm, healing garden, barber shop, music shop, etc. The role focuses on assessing residents' abilities and interests, developing individualized vocational roles within the nursing home and beyond, including sourcing and securing suitable job placements externally for residents who demonstrate potential for external roles in open or supported employment.
 
Job Responsibilities
Assessment & Planning
Conduct vocational assessments to evaluate residents' skills, interests and work readinessIdentify barriers to participation and develop tailored vocational rehabilitation plansCollaborate with families, care teams and program staff to ensure holistic supportGuidance & Coaching
Provide one-on-one and group coaching to enhance residents' confidence, motivation and social skillsSupport residents in goal-setting, vocation exploration and overcoming personal or functional challengesOffer guidance and training on workplace behaviour, task management and interpersonal skillsProgram Integration & Support
Facilitate residents' participation in Oasis Village (minimart, coffee shop, pet farm, etc)Coordinate with Job Coaches and Program Executives to ensure smooth vocational activities and monitor residents' progressRecommend accommodations or modifications to tasks based on residents' abilitiesCo-develop SOPs and workflows together with the other stakeholders to ensure seamless service delivery to clientsEngage and liaise with external parties to expand opportunities and provide support to residents who are able to engage in vocational arrangementsDocumentation & Reporting
Maintain timely and accurate records of assessments, vocational plans and residents' progressReport outcomes to management and propose improvements for vocational programsCollaboration & Training
Work closely with the multidisciplinary team including clinical staff, therapists, therapist assistants, job coaches and program staff to provide integrated supportConduct workshops or training sessions for staff on vocational rehabilitation principles and resident engagement strategiesGeneral Duties
Any other duties as assigned by the Program Manager and / or the Executive Director.
