Kidney Care Coordinator (Contract / 5.5 Days / North) (ID : 678006)

Job Responsibilities :

• Coordinate and manage the care of patients diagnosed with chronic kidney disease
• Conduct patient education and lifestyle counselling sessions to promote better self-management
• Provide support and guidance to caregivers and family members
• Collaborate closely with physicians, nurses, and allied health professionals to ensure continuity of care
• Maintain and update patient records, databases, and care plans accurately
• Track patient progress and support follow-up scheduling and compliance
• Prepare and submit timely reports to the Programme Manager and / or Renal Champion Doctor
• Ensure adherence to infection control and safety standards in all work environments
• Contribute to the development and improvement of patient education materials
• Participate in the review and enhancement of clinical workflows and care protocols
• Provide guidance and peer support to newly onboarded team members when needed
• Participate in internal audits, quality improvement projects, and programme evaluation
• Undertake any other tasks or assignments as directed by supervisors or programme leads

Job Requirements :

• Bachelor's Degree in a Science-related field like Biomedical Science, Life Sciences
• Prior experience in healthcare, patient education, or chronic disease management is an advantage
• Comfortable working both independently and in a team-based clinical environment
• Strong organisational skills and attention to detail in maintaining patient data and reports
