Job Responsibilities :

Responsible for the provision and management of nursing care to patients, providing supervision, guidance and direction to junior nurses and nursing students to ensure that optimal care is provided. The individual should be a good team player who is knowledgeable, resourceful, systematic and highly motivated.

Administer self-management plans to empower patients, family members and caregivers to facilitate participation in their own health management

Deliver evidence-based care in collaboration with inter-professional

teams

Facilitate admission, transfer and discharge of patients

Facilitate transition of care coordination within and between teams and across settings

Implement clinical risk and quality improvement initiatives and activities

Job Requirements :

Diploma or Degree in Nursing

Registered with LJM and have valid APC (2025)

Freshies welcome (less than 1 year Experience )

Experienced Nurses from all disciplines are welcomed

Proficient in English

Basic PC skills - Excel / Word / Emails etc.

Good Health with Covid vaccinations
