Job Responsibilities
Casework and Counselling
- Maintain at least 80% of active cases per quarter of which 100% to be supported with electronic recordings
- Conduct assessment, planning and intervention for cases
- Participate in internal and external case conferences, networking and case management meetings
- Document and evaluate case outcomes
- Develop and review appropriate intervention strategies
- Adhere to CSWP and SSNet processes in managing cases
Information and Referral
Provide accurate information and refer clients to appropriate agencies for further interventionAttends to walk-in clientsAdhere to CSWP and SSNet processes in managing referralProgramme – Groupwork / Community Work
To provide support to Coordinator in planning, implementation, evaluation and documentation of the programmeTo plan and organise events and activities for clientsQualifications and Requirements
Degree or Post-Graduate Diploma in Social WorkMin 3-5 years relevant work experience in Social Work settingDemonstrate ability to handle work professionally, independently and efficiently with a strong customer service mindsetPossess effective organizational and follow-up skills, paying attention to detailGood interpersonal skills and able to interface with all levels, internally and externally, in a professional and responsive manner.Tell employers what skills you have
