This role will support the HOD to oversee the service
planning and development, implementation and review of primary
care-led mental health programmes under the Community Mental Health
(CMH) Masterplan and HSG MH initiative and oversee the development
of HSG MH and dementia :
Support the
HOD in the strategic implementation of services in the primary care
setting focusing on mental health, and dementia
services.
Present findings including potential
strategies to senior management, to secure buy-in for policy review
and service planning.
Build trust and leverage
on internal and external stakeholders to identify gaps and
opportunities to improve service planning for mental health
services in the primary care.
Integrate mental
health primary care-led services into the broader primary
healthcare system, in collaboration with the Clusters' Regional
Health System Offices (RHSOs), the Primary Care Offices (PCOs) and
/ or the Primary Care Networks
Review and
establish competencies, structural processes and enablers to
support the implementation of the HSG MH in the private and public
primary care sector
Plan the implementation of
care model in tandem and aligned with the Healthier SG care
protocol, including continuous review and enhancement of the
collaborative care approaches to meet the intended outcomes and
support the needs of the clients and caregivers.
Manage budgets and monitor programme effectiveness
through evaluation efforts by coordinating, collating and tracking
the outcome measures and performance indicators of the primary care
programmes.
Develop a mental health in primary
care system such that the polyclinic and GP settings use the same
assessment tools, with the same understanding (language used) of
the MH / dementia needs of the patients.
Review
effectiveness of primary care programmes, including outcomes and
learning points for long term programme
planning.
Job
Responsibility
Bachelor's Degree in healthcare related discipline, with
preference for mental health qualification.
At
least 10 years' experience in primary care or community development
and multi-agency work in healthcare / social care industry with 5
years' management experience
Experience in
multi-agency work / healthcare industry, programme implementation
and / or programme management
Experience and
knowledge in facilitation, design thinking, organization
development preferred
A self-starter with
strong learning ability, who is resilient, adaptable and able to
manage change and ambiguity
Able to multi-task
and possess strong analytical, problem solving and project
management skills
Able to work well with
groups from diverse background
Experience and
knowledge in facilitation, with strong negotiation and
communication skills
