You will be part of the team in coordinating and managing the care of chronic kidney disease patients.
PRINCIPAL RESPONSIBILITIES
- Evaluate, plan and implement care of patients with chronic kidney disease.
- Conduct teaching and counseling for patients and their families / carers.
- Coordinate patient care where required.
- Collaborate and co-operate with physicians and other health-care providers in the delivery of patient care and services.
- Maintain related database.
- Provide continuous and relevant updates, and submission of reports to the Programme Manager and / or Renal Champion Doctor where required.
- Maintain a clean and safe working environment.
SECONDARY RESPONSIBILITIES
Assist to review and develop patient education materials in consultation with physicians or other health-care providers where required.Assist to review and evaluate work processes in consultation with physicians or other health-care providers where required.Precept newly joined coordinators in delivery of patient care where needed.Participate in quality improvement activities.Take on additional responsibilities or tasks as assigned by the Reporting Officers.REQUIREMENTS :
Bachelor Degree in Life Science, Medical Sciences and / or related medical fields.Proficient in Microsoft applications.Team player with excellent interpersonal and communication skills.
