Roles & Responsibilities
Job Responsibilities :
- Conduct Initial Assessment and using relevant outcome measures
- Design Individualised care Plan and goal settings
- Provide treatment intervention at home setting
- Caregiver training and equipment prescription
- Discharge planning and refer to other services if required
- Amends monthly home therapy meeting
- Do therapy inservice and attend the training organised by the department
- Handle other admin duties
Job Requirements :
Degree / Diploma in Physiotherapy recognised by Allied Health Professions CouncilFull registration with Allied Health Professions CouncilGood interpersonal skillsIndependent in conducting assessments and intervention at home settingDesign the individualised care plan tailored to each patientOnly shortlisted candidates will be notified.
