RESPONSIBILITIES

1. Support team in monitoring of seniors' physical and mental health and well-being :

• Conduct visual check and take / monitor vital signs
• Act on clients' irregular conditions, report to senior care staff or supervisor and caregiver, if necessary, record in communication notes, submit incident report where needed
• Identify changes in seniors' well-being / behaviours, report to caregivers, plan and monitor Individualised Care Plan (ICP)  with Physiotherapist or Occupational Therapist.
• Respond to emergency and accidents. Use of PPE if required
• Administer medications following SOP, after being trained

QUALIFICATIONS

Minimum 1 year of working experience in healthcare / eldercare setting is preferred or possess any of the following :
