My client is seeking an experienced healthcare professional
to lead a team focused on post-discharge care coordination and
community integration. This role involves strategic planning, team
leadership, and cross-sector collaboration to improve patient
outcomes and reduce unnecessary hospital
readmissions.
Key
Responsibilities : Lead
and mentor a team of care coordinators supporting patients and
caregivers after discharge
Oversee follow-up
care through phone outreach and home visits, ensuring patients can
safely transition back into the community
Coordinate with healthcare partners to streamline
services and strengthen support systems
Develop and implement frameworks that enhance community
care delivery
Supervise clinical performance
and guide professional development for team members
Monitor data quality and contribute to service
improvement initiatives
Participate in
internal projects to improve care standards and operational
efficiency
Requirements :
Professional background in Nursing, Social Work, Allied
Health, or related clinical fields
Minimum 8
years of experience in care coordination or community
health
Proven leadership experience (3-5
years) managing multidisciplinary teams
Strong
communication, planning, and stakeholder engagement
skills
