The incumbent plays a vital role in
delivering holistic care to our patients, extending beyond
rehabilitation.
Develop, promote, and
implement wellness initiatives and activities at the community
dialysis center.
Oversee patient screenings,
conduct physical assessments, compile and analyze data, and provide
recommendations for follow-up care.
Conduct
routine reviews to track patient adherence and progress in the
exercise program, ensuring continuity of care plans.
Ensure proper maintenance, disinfection, and servicing of
exercise and program equipment.
Monitor and
manage patient attendance, ensuring accurate and timely
documentation to support consistent program
participation.
Work collaboratively with the
Clinical Services Team to identify and address barriers to
participation, ensuring patients receive adequate support in their
wellness journey.
Support the planning and
execution of volunteer events and social outings to empower and
engage senior
patients.
Requirements
Prefarable Diploma in Health Sciences (Health Management)
or other relevant courses (e.g., sports sciences, therapy aide,
community care associate).
Relevant working
experience in a healthcare setting is
advantageous.
