Social Worker / Counsellor (Community Outreach)

Job Summary :

As a Social Worker / Counsellor, you will be responsible for organizing and conducting outreach to increase mental health awareness. You will support clients who are at risk of mental health issues by providing mental health screening and assessments, and by linking them to the necessary support and resources. You will continue to monitor these clients in the community.

You will also be involved in data collection and evaluation of the CREST programme.

Principal Responsibilities and Duties :

1. Programme Development and Implementation

• Provide care coordination and service linkage to clients with or who are at risk of mental health concerns
• Plan and conduct outreach to the community and stakeholders including faith-based, corporate and community organisations to promote mental wellness and mental health awareness.
• Conduct assessment (i.e. Mental Health needs, behavioural, developmental, risk, safety)
• Provide counselling, case management, and psychosocial interventions
• Conduct active monitoring and maintenance of clients
