Job descriptionMust have a Degree or Post Graduate Diploma in Social Work or Masters in Social Work Minimum 3-year experience working in the social service sector, preferably in the senior care sector A team player and able to work independently Passionate about serving the social service sector to provide better services and care to the clients Possesses good attitude with good interpersonal and communication skills Bilingual and dialect speaking preferred to liaise with dialect speaking clients Proficient in Microsoft Office Open to Singaporeans only

Responsibilities
- To provide case management / supportive counselling for seniors and their family caregivers. The social worker is to conduct psycho-social and financial assessments, including para-counselling. Makes appropriate recommendations and to carry out an effective clinical intervention for the clients.
- To plan, develop and implement programmes / services according to the needs of the seniors / community and family caregivers.
- To plan and facilitate in caregiver support group / group work
- To conduct home and hospital visits
- To network and liaise with stakeholders, relevant agencies, and organisations regarding to the welfare of the seniors and their family caregivers
