Medical Social Worker

Job descriptionBe part of a patient-centred, multidisciplinary team supporting holistic healthcare delivery Work across diverse settings such as acute hospitals, community hospitals, outpatient clinics, and home care services Opportunities for clinical supervision, continuing education, and professional development Make a meaningful impact on lives through direct patient support and advocacy Conduct psychosocial assessments to understand patients' emotional, social, and environmental needs. Provide counselling and supportive therapy to patients and families coping with illness, grief, trauma, or major life transitions. Develop and implement individualized care plans in collaboration with healthcare teams to ensure integrated support. Assist patients in accessing financial assistance schemes, community resources, and social services. Advocate for patients' needs, rights, and well-being in care planning and discharge arrangements. Participate in case discussions, multidisciplinary meetings, and family conferences. Provide crisis intervention, especially in cases involving abuse, suicide risk, or sudden illness. Maintain accurate and confidential case documentation in compliance with institutional and ethical standards. Degree or Postgraduate Diploma in Social Work from a recognized institution Eligible for registration with the Singapore Association of Social Workers (SASW) We regret to inform only shortlisted candidates will be notified.
