Homecare Staff Nurse (5days work week, Good working environment) #HCL - Islandwide, SG

Roles & Responsibilities

Key Responsibilities

• Assess new clients and develop personalised care plans.
• Perform nursing procedures (wound care, tube / catheter care, stoma care, medication management, monitoring vital signs).
• Conduct health screenings and make referrals when needed.
• Provide health education to clients and caregivers.
• Coordinate care with doctors, allied health and external partners.
• Build rapport with clients and families.
• Support self-management of health conditions.
• Provide tailored health education.
• Case Management & Administration
• Plan and facilitate Homecare services with the team.
• Maintain accurate client records and reports.
• Manage client workload through scheduling.
• Recognise early warning signs of deterioration.
• Provide appropriate emergency or crisis care.
• Handle enquiries about Homecare services.
• Monitor and manage medical supplies and equipment.
• Support training and development of community partners and volunteers.
• Assist in community outreach and partnership activities.
