Mobile Health Screening Nurse
Roles & Responsibilities
About the role
This is a nursing position focused on providing comprehensive care and support for patients with chronic diseases including diabetes, hypertension and hyperlipidaemia. You will deliver patient education on disease management, lifestyle modification and medication adherence, while conducting health screening and clinical assessments across multiple clinic settings.
Key responsibilities
Provide nursing care and counselling for patients with chronic diseases such as diabetes, hypertension and hyperlipidaemia
Provide patient education on disease management, lifestyle modification and medication adherence
Conduct health screening and basic clinical assessments
Independently perform venepuncture/blood taking, ECG, treadmill testing, diabetic foot screening and diabetic retinal photography
Support and independently run health screening services at clinic, pop-up clinics and external onsite locations
Manage nursing documentation, Clinical Management System (CMS) and basic clinic administration
Recognise abnormal findings and escalate appropriately to the doctor
About you
Registered Nurse with at least 3 years of relevant clinical experience, preferably in primary care and health screening
Confident and competent in venepuncture, nursing care and chronic disease counselling
Experience in diabetic screening, chronic disease management and patient counselling preferred
Comfortable using a Clinical Management System (CMS) and handling clinical documentation
Able to work independently with minimal supervision and take ownership of assigned duties
Strong attention to detail, accuracy and clinical judgement
Well-organised and comfortable handling clinical, administrative and logistical tasks
Comfortable working across clinic, pop-up and external onsite health screening locations
