Founded in 1976, Home Nursing Foundation (HNF) is a leading charity that provides home healthcare services in Singapore, regardless of our patients' ability to pay. Dedicated to all-round support for our patients, HNF delivers comprehensive care programmes to look after patients' emotional well-being and healthcare needs.Our HNF Wellness Day Care & Rehabilitation Centres offer an array of social activities, social support and holistic healthcare services to help our elderly stay independent in their own community.

RESPONSIBILITIES

• Lead and support ACP discussions, helping clients explore and completing advance care meet clients' needs.
• Document and update client ACP preferences for submission accurately, ensuring proper record-keeping with proficiency in administrative processes
• ACP Facilitators can either source their own leads or receive leads
• Provide feedback on ACP services for quality improvement

QUALIFICATIONS

• Certification in ACP Facilitation is required, but candidates without the General ACP certification may be considered.
• Experience in healthcare or community settings, particularly in facilitating ACP discussions is a plus.
• Able to manage multiple cases and prioritize tasks effectively
• Strong communication and interpersonal skills to guide sensitive discussions with clients and families
• Fluency in English and ability to communicate in local dialects will be advantageous (to liaise with local dialects clients)

OTHER INFORMATION

• Flexible working hours based on client availability. Sessions can be virtual or onsite at HNF offices, assigned venues (North-East and Central) or client locations. Travel may be required for client sessions and events.
