Job Description

As a Community Nurse, you will:

• Work closely with a team of dedicated healthcare professional to administer patient-centered care
• Visit patients in their home to provide nursing care, educate patients and their families
• Manage Community Nurse Post one-day per week on roster basis
• Update records of patients' progression after each home visit
