Job descriptionVisit clients in their homes to provide occupational therapy services. Assess clients' needs, create care plans, and deliver personalised therapy. Work closely with caregivers, internal teams, and external partners to support client well-being. Keep accurate and timely records in line with professional and organisational standards. Monitor and evaluate client progress to ensure quality care. Coordinate part-time (locum) occupational therapy services when needed. Lead and support a team of Healthcare Assistants providing home care services. Degree in Occupational Therapy from a recognised institution. Fully registered with the Allied Health Professions Council (AHPC), Singapore. At least 2 years of experience in a hospital or community setting. At least 2 years of leadership or supervisory experience. Comfortable delivering therapy in clients’ homes and adapting to different environments. Organised, detail-oriented, and able to manage time well.
