Job Summary

• Key Responsibilities :
• Develop personalized care plans for residents to cater to their unique needs and preferences.
• Implement and monitor the effectiveness of these care plans, making adjustments as necessary to ensure optimal outcomes.
• Create and deliver engaging programs and activities designed to meet the diverse care requirements of our residents.
• Prepare and submit comprehensive review reports and interview records for all residents on a regular basis.

The ideal candidate will possess a degree in Social Work and relevant experience within the social service sector or a related field. A strong understanding of social work principles and practices is essential, as is the ability to work collaboratively with colleagues and stakeholders.
