About CCMS

Community Case Management Service (CCMS), developed by MOH and funded by AIC, supports frail seniors with multiple and complex care needs to age-in-place. As a Social Work Associate, you will conduct outreach and work with seniors, caregivers, and community partners to provide holistic and coordinated care.

Responsibilities :

• Outreach, network and build partnerships with internal (i.e. Fei Yue departments and programs) and external community partners (e.g. other social or community service agencies, hospitals) to identify potential clients for CCMS.
• Organise or participate in outreach events, roadshows, and talks to promote CCMS services.
• Assess clients’ psychosocial, medical, and functional needs.
• Develop and implement individualised care plans with clients, caregivers, and professionals.
• Coordinate and link clients to appropriate health and social care services.
• Provide caregiver support, education, and navigation of community resources.
• Monitor and review care plans to meet clients’ changing needs.
• Advocate for clients’ best interests and promote quality of life.

Work Location : Northwest and Northeast of Singapore
